Referral form

Referral must be arranged by your veterinary surgeon. We are unable to provide advice on pets we have not examined.

Please ensure pets are safe to travel particularly if they are struggling to breath, collapsed, painful or not eating. These cases should be assessed at the primary vets or their out of hours provider. It may not be safe to wait for a referral appointment. Complex cases in which there is concern regarding non cardiac illness may benefit from being seen at a multidisciplinary centre.
